Student Registration

All children entering Queensbury UFSD, including those returning to the school district, are registered at the central registration office located in the administrative office building at 429 Aviation Road.

Please note: If you are transferring a child into the district in grades 1-12, with an anticipated start date of September 2021, the District Registrar cannot accept transfer documents until after July 1, 2021. Thank you.

Kindergarten

Grades 1-12

  1. Preregister your child (or children).
  2. Download and complete the registration packet.
  3. Email the completed packet and the required documents listed below to lmelleon@queensburyschool.org.
  4. Complete the free and reduced-price meals application.
  5. Contact our district registrar to schedule an appointment at (518) 824-5616.

Required documents

  • Proof of date of birth is required
    • Please provide your child’s original birth certificate (we will make a copy here).
    • If your child’s birth certificate is unavailable one of the following can be used for identification: Original passport (we will make a copy here), Military Dependent ID Card or Certified record of baptism.
  • Proof of residency is required
    • If you own your own home: Please provide a bill or statement dated within the last 60 days with your name and home address pre-printed on it. It can be one of the following:
      • Telephone bill
      • Cable bill
      • Utility bill
      • Car or Home Insurance Policy
      • Bank/Credit Union Account Statement
      • Pay Stub
      • Tax Bill
    • If you are renting: Please provide a current original signed lease which includes the landlord’s name, address and telephone number.
    • If you are moving into the district and are in the midst of purchasing property: Please provide an original signed contract to build or buy dated within 90 days, which includes buyer’s and seller’s names along with the name and address of the realtor. You will then have 30 days from your move-in date to provide a second proof of residency from 2a above.
  • Immunization record & recent physical information
  • Completed registration packet listed above
  • Parent/guardian valid photo ID

If applicable

  • Custody papers (must be a signed copy)
  • Foster care letter
