There are two ways students can obtain working papers:

1. Complete part one of the online application (do not complete any other parts)

2. Provide a copy of the student’s physical (must be within one year)

3. Email the completed application and the physical to the applicable individual:

   1. Student last name A-M: Contact K. Nelson

   2. Student last name N-Z: Contact S. de Jesus

1. Request an application be mailed to you directly by calling (518) 824-4604. 

2. Provide a copy of the student’s physical (must be within one year)

3. Mail the completed application and the physical to:
   QHS
   ATTN: Counseling Center – Working Paper Application
   409 Aviation Rd
   Queensbury, NY 12804 

The counseling center will then:

• complete the working card

• mail the working card to your home address

Please note that this may take up to two weeks to process.
